
Lower back pain is one of the most common ailments worldwide, a symptom that, although most experience it at some point, can become a source of significant disability. Globally, and particularly in Spain, it is the leading cause of work limitations in adults. It is important to understand that low back pain, rather than a disease in itself, is a symptom with multiple origins. While acute episodes of lower back pain usually improve on their own within a few weeks, recurrences are very common, and a considerable portion of the population experiences persistent pain.
When faced with chronic or recurrent low back pain, the main challenge is finding a treatment that is effective in the long term. In this context, physical exercise stands out as one of the most studied treatments with the greatest evidence of efficacy, improving both pain and functionality. Unlike quick fixes, such as surgery or certain medications (anti-inflammatory drugs or antidepressants), which often carry side effects and do not always outperform more conservative therapies, exercise offers a sure way to strengthen the back and promote lasting recovery. In fact, studies show that surgery, for example, is not significantly more effective than conservative treatment based on exercise and education.
However, for exercise to be truly beneficial, it is crucial that certain conditions be met. First, these programs are intended for nonspecific low back pain, that is, pain whose origin is not linked to a serious cause (such as a fracture, tumor, or infection). Often, findings on imaging tests, such as MRIs, are so common in people without pain that a direct cause-effect relationship cannot always be established. Furthermore, exercise is best suited for chronic or recurring pain of moderate intensity. If the pain is very severe, it is advisable to take steps to reduce it before starting.
Finally, the success of treatment depends largely on the patient's commitment. Exercise, although with very few side effects, does not offer immediate results. It requires motivation and commitment to perform the movements independently and consistently for several months. Combined with other therapies, such as medication, exercise becomes a fundamental pillar of a comprehensive treatment that restores control and well-being to your daily life.
