Viscosupplementation is the intra-articular application of viscoelastic substances to improve synovial fluid or replace it with a higher-quality fluid. Viscosupplementation is used as an alternative treatment in the non-surgical management of osteoarthritis.

If we assume that osteoarthritis is a degenerative, irreversible, and progressive process, we must accept as its primary objective the improvement of patients' functional quality through pain relief and return to daily activities.

Viscosupplementation (hyaluronic acid)

Hyaluronic acid is a polysaccharide chain formed by glucosamine disaccharides and glucuronic acid. Hyaluronic acid reduces inflammation, pain, and protects the remaining hyaline cartilage. Furthermore, it stimulates the synthesis of higher-quality synovial fluid.

It is administered by joint puncture and can be performed in an office setting under strict aseptic conditions. Cortisone injections can be performed beforehand (one week) to reduce synovial activity, followed by hyaluronic acid injections.

The patient must be informed at all times of the temporary nature of the improvement with hyaluronic acid injections. Symptom improvement can range from 6 to 12 months, and the treatment can be repeated as many times as necessary.

Therefore, viscosupplementation, through intra-articular injections of hyaluronic acid, is a useful tool for pain management in patients who do not wish to undergo surgery or wish to delay surgery as long as possible.
